W-2 Laser Sets
In the states where there isn't a State or Local Income Tax, the basic 4-part form is sufficient. Most States that have a State Income Tax Program require a 6-part form. The 6-part form comes with an additional copy for the employer to submit to the state, and an additional copy for the employee to attach to their State Tax Return. In States that have a Local or City Tax, an 8-part form is required.
